This chapter promotes understanding of End-of-life communication through structured communication tools supports consistent and effective goals-of-care discussions by clarifying values, treatment preferences, and advance directives. Nurse and health care professionals will learn patient centered and team focused communication and interprofessional collaboration skills and approaches utilizing empathy, active listening, cultural sensitivity, clarity, and honesty to promote alignment among patients, families, and the interprofessional healthcare team during the end of life.
