In this formal review, the challenges of delivering therapeutic agents for blood-borne cancer to the brain microenvironment are discussed. The complexities of the blood–brain barrier (BBB) and the blood–tumour barrier (BTB) are highlighted, including their non-uniform permeability and active efflux of molecules. Strategies to improve drug delivery across the BBB and BTB are addressed, focusing on the impact on conventional and emerging treatments. The potential of new personalized treatment strategies for primary brain malignancies and brain metastases is also emphasized.
