I have used this for clients' "Evaluation of Coaching". It is simple and much easier than designing forms using Microsoft Word or Access. Although the free version only allows ten questions, most responders do not want to answer long questionnaires. Using different types of questions, such as rating scales, allow multiple areas/rows to inquire.
