Individualized Transition Plans: Practical Resources
Individualized Transition Plans (ITPs) are now required for students who have IEPs ages 14 and up. Goals need to be developed in such areas as education, employment, and independent/community living.

1. Divide the class into small groups of 4 to 5 students.
2. Have one group member describe a secondary student with whom he or she has
recently worked.

goals in the areas of: (1) education, (2) employment, and (3) independent/
community living.
